Your Compliance Checklist

 

Use this to make sure you do what you need to do to finish getting into compliance. 

1.  Get your driver's pre-employment drug screen done ASAP

You will be getting an email from I3Screen.com with a form called a donor pass attached with the location and contact information to get your driver's pre-employment drug screen completed. You can either print out and take take the donor pass or open the attachment and show the collection site staff. The driver will not have to pay for this drug screen at the site. Your result will be forwarded via email when available.

Please note if your pre-employment drug screen order expires before it's completed:

If a driver fails to get to the collection site and complete the urine collection by the expiration date, the screening must be reordered and paid in full to replace the original screening.

2. You must register at the FMCSA Clearinghouse website ASAP

Please note: The FMCSA Clearinghouse is a government website and not affiliated with mydrugtestingprogram.com. We cannot perform your clearinghouse services until you complete your registration at the FMCSA Clearinghouse. 3. Each of your drivers (including owner operators) must be registered as drivers at the FMCSA Clearinghouse.

4. You'll also need to have each of your drivers fill out a clearinghouse annual limited query consent form, and keep a copy in their file. (Click here for the form).

5. You are required to run a pre-employment query and get a "not prohibited" result on each driver at the clearinghouse before the driver is allowed to perform safety sensitive duties (drive a comercial motor vehicle under these regulations.)

6. Drivers must complete the pre-employment query with the Clearinghouse, by logging into their clearinghouse driver account and providing their consent. 

All pre-employment queries (full queries) conducted at the FMCSA Clearinghouse will generate a consent request email (sent by the clearinghouse) to the driver. The driver will then be required to log in to their driver account at the clearinghouse and give their consent to complete the query. If a driver fails to do this they will be prohibited from driver until another query is competed with the driver giving their consent.

  • If your program with us includes clearinghouse services we will perform initial queries after we receive an alert from the clearinghouse that you designated us as your C/TPA.
  • Queries for new drivers added after the initial enrollment will be done after we receive a completed "add driver" form via email.

7.  Required annual limited queries must be run on existing drivers once a year.

  • If your program with us includes clearinghouse services for a driver we will perform yearly queries for each active driver in the program.

8.  Complete your required driver training (owner operators and employed drivers.)

9.  Complete required supervisor training (If you have CDL drivers that are not owners of the company) Follow the instructions in the members resource area located here.

10.  Set up private files in a secure private location for each of your drivers and supervisors.

11.  Paper drug screening forms will sent to you by FedEx to the address you provided us.

  • These are for backup as we will be setting all your screening up electronically.
  • You must notify Mydrugtestingprogram.com before using a paper form for drug screening.
  • Keep them in a location where you will remember where they are.
  • Have each driver keep a couple in their trucks in case testing needs to be done while they're on the road. 

12.  Keeping your driver list up-to-date with us

  • Drivers who will be inactive must be removed from your list.
  • Drivers who have been terminated must be removed from your list.
  • You will be asked to verify your driver list quarterly.

13. How to add drivers to your program

Adding drivers (beyond the ones you started with when you enrolled in our services) requires you complete and send an add driver form to us. You will choose a package based on what services you want for each driver.

14. Audits and required reports

If you are notified of an audit or request for any document by federal or state authorities, you must contact us immediately and send us a copy of the notice so we can help you assemble any requested documents related to your drug and clearinghouse services programs. Send audit notices to admin@mydrugtestingprogram.com.

  • Audit assistance is included with your program but
  • Audit requests made with 5 days of an audit may require an expedited audit assistance fee of $100, so get it to us ASAP.

15.  When setting up drug screening we will use Quest Laboratory Patient Service Centers (PSC) and Quest Preferred Provider Network sites (PPN), or one of the 10,000 plus  in-network sites that accept Quest forms. 

16.  Yearly program and driver renewals

You will receive advanced notice that your program or added driver yearly renewal is coming and a billing date when we will debit the credit or debit card you provided us.

  • You should contact us prior to the billing date to make changes to your card on file, let us know a driver is no longer active, or to terminate your service.
  • A payment failure will result in your program being made inactive until the amount due is paid.
